Mentoring
The next step after mental health or addiction treatment is often challenging to navigate. Our mentors can help clients seamlessly transition back to everyday life during and after treatment so they feel confident that the next step is the right one.
Camden Case Management works with clients to develop a comprehensive plan to reintegrate back into their home, work, and social lives. We assign clients both a mentor and case manager to facilitate the process.
By working with a multidisciplinary team, clients can receive the insight needed to develop a plan that works for them, along with ongoing support and communication to make sure they continue to reach their goals.
Mentoring Services
Mentoring services at Camden Case Management include:
- Regular check-ins with a mentor, including in-person and virtual meetings, daily phone calls, and home visits
- Care coordination and communication with treatment providers and family members, in conjunction with a case manager
- Life skills support
- Development of healthy outlets and wellness-related activities
- Medication management
- Job coaching, such as career exploration, resume writing, and help with job searches
- Toxicology screening (at an added cost)
If challenges arise in a client’s healing journey, our mentors also provide resources, support, and crisis intervention services to help get them back on track.

Our mentors operate in collaboration with the client’s outside clinical teams in forming a united front, helping our clients to develop the agency necessary to launch and utilize the skills and resources they cultivate in their clinical sessions.
