Our Case Manager Only Support is ideal for clients who seek structured guidance to stay on track with their goals but don’t require intensive, in-person services. Case Manager Only Support connects you with a dedicated case manager who provides goal-oriented planning, regular check-ins, and seamless coordination with your clinical team—all while respecting your independence.
What This Includes:
- Goal Tracking & Accountability: We work with you to set achievable goals and provide regular check-ins to keep you on course, reinforcing motivation and accountability.
- Coordination with Your Clinical Team: Your case manager ensures clear communication with your clinical team, aligning all professionals toward your objectives for seamless, coordinated support.
- Flexible, Low-Intensity Support: Designed for clients who prefer a light-touch approach, this service offers the right amount of support without the need for frequent in-person sessions, allowing for greater independence.
- Customized Planning: Your case manager collaborates with you to build a tailored plan that addresses your unique needs without overwhelming your schedule or adding unnecessary services.

Case Management with Mentor and Companion Support
Our Clinically-Informed Case Management with Mentor and Companion Support offers a flexible, comprehensive approach, providing clients with tailored assistance to meet their unique recovery needs. Through coordinated mentorship and companionship, clients receive a structured foundation designed to help them achieve their goals with the backing of a dedicated multidisciplinary team.
What This Includes:
- Companionship for Intensive Support with Step-Down Program: Designed for clients who need significant, hands-on assistance, companionship is available from 1 to 7 days per week, with options for 24, 18, or 12 hours per day. Our step-down program allows for a gradual reduction in hours as clients progress, easing the transition to independence with clinically guided adjustments.
- Mentorship for Moderate Support (Minimum of 2 Hours): Ideal for clients who are more independent but benefit from structured guidance, mentorship provides flexible support from 1 to 7 days per week, with sessions ranging from 2 to 11 hours per day. This option focuses on enhancing executive functioning, relational skills, and daily task management in a way that aligns with each client’s goals.
- Flexible, Weekly Adjustment: Both companionship and mentorship services are adaptable on a weekly basis, ensuring that the level of support can increase or decrease based on each client’s evolving needs, as guided by clinical recommendations.
- Personalized, Goal-Oriented Plans: Mentors and companions collaborate with clients to create individualized plans that address activities of daily living (ADLs), executive functioning, hobbies, and relational skills, ensuring alignment with the client’s overall treatment objectives.
- Clinical Oversight, Family Support, and Continuous Communication: Each client is supported by a multidisciplinary team, including Licensed Marriage and Family Therapists (LMFTs) and case managers who provide weekly supervision. As part of our commitment, families have access to a Family Support Group, offering resources and a supportive space to stay engaged in their loved one’s journey. Regular communication with the family and clinical team ensures all members are informed and aligned on the client’s needs, progress, and goals.
Our goal is to empower clients and their families with a holistic, supportive care system. With Clinically-Informed Case Management and the added flexibility of mentorship or companionship, clients experience a seamless, adaptable path toward independence and well-being, backed by a team that’s invested in their long-term success.
30/60/90 Companionship Packages
Our 30/60/90 Companionship Packages are curated for clients who are navigating particularly intensive periods in their lives and need substantial, hands-on support. With the addition of a dedicated case manager, each client receives not only the direct assistance of their companion team but also personal case management oversight to guide their journey.
These packages are ideal for clients facing life transitions, early sobriety, undergoing complex medical conditions, emotional challenges, or unpredictable times. Each client is paired with a consistent team of companions to build rapport and trust, fostering a deep understanding of their needs and goals. Through Camden Case Management’s collaboration with the client’s recovery team and family, companions are well-prepared to provide highly personalized, effective support.
The structure of the packages offers three distinct phases, each supervised by a clinically-informed case manager to ensure the right level of support at each stage:
- Stabilize Phase: 24 hours of support per day during the first 30 days, delivering intensive assistance during the most challenging times.
- Launch Phase: 16 hours of support per day over the following 31-60 days, helping clients stabilize and establish routines.
- Thrive Phase: 8 hours of support per day in the final 30 days, supporting clients as they move toward greater independence.
Clients can choose to experience each phase daily for 30, 60, or 90 days, or progress through a phased approach to suit their unique needs and goals.
Specialized Mentor Only Support
Our Specialized Mentor Only Support service connects clients with highly qualified Specialized Mentors, each skilled in various clinical and therapeutic techniques. This service is designed for clients who need focused, personalized guidance on specific challenges without requiring the comprehensive oversight of a case manager. Every Specialized Mentor is carefully matched to meet each client’s unique needs, delivering targeted support aligned with their goals.
What This Includes:
- Qualified, Specialized Mentors: Our team of Specialized Mentors hold specific qualifications tailored to address the unique needs of each client. These mentors bring expertise in clinically-informed and therapeutic techniques, providing practical, skills-based guidance that helps clients navigate specific challenges and achieve meaningful progress.
- Focused, Client-Centered Approach: This service emphasizes direct, individualized support on specific issues or goals the client faces. Without the additional external coordination of a case manager, the Specialized Mentor is able to concentrate entirely on the client’s needs, offering intensive, one-on-one guidance and specialized support.
- Regular Progress Updates: Specialized Mentors provide consistent updates to caregivers and the client’s clinical team, ensuring alignment with evolving goals and maintaining clear communication regarding the client’s progress and needs.
- Comprehensive Support Network: Although this service does not include a case manager, Camden Case Management upholds its commitment to a full, multidisciplinary support system behind each client. This includes access to Licensed Marriage and Family Therapists (LMFTs) and a multilayered team, offering oversight and backup, so clients benefit from a comprehensive foundation of support.
- Family Support Group Access: Family members are encouraged to participate in our Family Support Group, providing them with resources, guidance, and a supportive community throughout the client’s journey.
Our Specialized Mentor Only Support service offers a flexible, tailored solution for clients who require focused, one-on-one mentorship without the broader coordination of case management. Backed by Camden Case Management’s commitment to holistic support and family involvement, this service provides expert guidance for specific goals, empowering clients to make meaningful progress with the dedicated support of a Specialized Mentor.
Case Management Medical Advocacy Program (CMMAP)
Our Case Management Medical Advocacy Program (CMMAP) is designed to provide specialized support to clients experiencing complex medical conditions such as pain management, chronic fatigue, pain management, etc. CMMAP Advocates assist those who are navigating complex medical journeys, particularly those undergoing advanced medical treatments. Through CMMAP, clients are connected with an experienced medical advocate who brings both knowledge and compassion to guide them through challenging health experiences.
What This Includes:
- Experienced, Specialized Advocacy: Each client is paired with a highly trained medical advocate who understands the intricacies of medical environments and treatment protocols. This advocate is uniquely qualified to comprehend the client’s needs and struggles, providing critical support and empathy during difficult times.
- Personalized Support & Coordination: Acting as a bridge between the client and their medical and clinical teams, the advocate ensures that every aspect of care is smoothly coordinated. From managing appointments and treatment schedules to facilitating communication between healthcare providers, the advocate streamlines the client’s medical experience, allowing them to focus on their recovery.

Executive Coaching

This coordination ensures that transportation is not only safe but also aligned with the client’s treatment and support needs.
