The Camden Case Management Model

The Camden Case Management Model is designed to provide a bridge between clinical treatment and real-world application using a unique and clinically-informed approach. Our model is centered around close collaboration with a client’s clinical team, translating a client’s experiences outside the treatment room into actionable insights for clinicians, and providing support for applying therapeutic gains to a client’s day-to-day life. This approach provides additional and enhanced information, allowing clinicians to adapt and tailor treatment as a client’s needs continue to evolve.

Our staff is specifically trained and supported to work within this model. Each member undergoes foundational and ongoing training and participates in weekly case reviews with a licensed mental health professional, equipping them with the skills to observe, interpret, and respond to clients’ needs through a clinically-informed lens. This means that each member of our team is an astute discerner of behaviors, triggers, and progress markers, allowing us to effectively communicate these observations back to the clinical team. Our insights can meaningfully inform the client’s therapeutic plan, facilitating adaptive, responsive care.

For clients facing mental health, addiction, and medical challenges, the Camden Case Management Model offers the same level of clinically-informed, tailored support across all our services. Our team works closely with clinical and medical teams to track and report client progress and challenges in real time, creating a coordinated care environment that adapts to each client’s evolving needs. We focus on providing clinically grounded techniques to guide clients through high-stress situations, ensuring therapeutic gains are integrated into their daily lives.

Each of our services is specifically designed to function within this clinically integrated model. Our team members are trained to understand the clinical and medical goals for each client and reinforce their constituent steps in practical, real-world settings. This approach ensures that therapeutic gains aren’t confined to the treatment room, but instead become part of the client’s everyday experience.
