The 30/60/90 Companionship Program provides time limited, highly structured, in person support for individuals who need a higher level of consistency and care during periods of significant challenge or transition. The program is designed to support safety, stability, and gradual movement toward independence through a phased approach.

This program is clinically informed and supported by an assigned case manager. The case manager coordinates care, holds the broader clinical and practical picture, and manages the many responsibilities that support safety, stability, and progress in daily life. Companionship is one part of this coordinated approach and does not operate on its own.

The 30/60/90 Companionship Program does not replace therapy. It works alongside existing therapeutic, recovery, and medical care to support continuity and alignment during periods of increased need.

Who This Program Is For

The 30/60/90 Companionship Program is appropriate for individuals who need sustained, hands-on support due to complexity, vulnerability, or major life adjustments.

We designed the 30/60/90 Companionship Program to provide the right level of support at the right time. While the program is organized around a 30/60/90 framework, the timing of each phase is guided by the client’s needs and how stability develops over time, rather than a preset schedule. Support can remain intensive when safety and containment are needed and is adjusted thoughtfully as skills are practiced and stability increases.

Each client is supported by a consistent team of companions to foster trust, familiarity, and continuity of care. Companions work under the direction of a dedicated case manager and with clinical oversight to ensure care remains aligned with the broader treatment plan.

Program Structure

Stabilize Phase

The Stabilize Phase focuses on safety, containment, and daily support during the most vulnerable periods.

  • Up to 24 hours of in-person support per day
  • Often used early in the process or during periods of acute need, with timing guided by the client’s stability rather than a fixed schedule
  • Emphasis on structure, monitoring, and consistent presence

While the program is organized within a 30/60/90 framework, the length of this phase depends on the client’s needs and how stability develops over time.

Launch Phase

The Launch Phase supports the development of routines and increased engagement in daily life.

  • Approximately 16 hours of in-person support per day
  • Often used as stability begins to increase, rather than tied to a specific day range
  • Focus on strengthening structure, skills, and daily participation

While the program is organized within a 30/60/90 framework, the timing of this phase is guided by the client’s needs and how stability develops over time.

Thrive Phase

The Thrive Phase supports growing independence while maintaining appropriate oversight.

  • Approximately 8 hours of in person support per day
  • Used as stability becomes more consistent, rather than tied to a specific day range
  • Focus on confidence, skill application, and gradual autonomy

While the program is organized within a 30/60/90 framework, the timing of this phase is guided by the client’s needs and how stability develops over time.

Clients may move through all three phases sequentially or participate in one or more phases based on individual needs, clinical guidance, and practical considerations.

What Companionship Includes

In Person Support Focused on Safety and Stability

Companionship is an in person support service centered on safety, stability, and containment. Companions provide steady presence and practical support during periods when clients need additional structure in daily life.

Daily Structure and Skill Building

Companions provide hands-on assistance with daily routines and practical skills that support stability and independence.

This may include support with:

  • Establishing daily structure and routines
  • Emotional regulation and grounding strategies
  • Social engagement and community participation
  • Medication reminders and wellness activities

Care Coordination and Safety Support

Companions monitor day-to-day safety and emotional well-being while maintaining close communication with case managers and Camden Case Management clinical leadership. When concerns arise, support can be adjusted quickly and thoughtfully.

This coordination helps reduce risk and supports continuity across settings.

Clinical Oversight and Family Communication

All Companionship services are supported by licensed clinical oversight and integrated within Camden Case Management services. Communication with families is coordinated and managed by the case manager, ensuring families receive regular, clear updates and remain appropriately involved.

This structure supports families while maintaining clear roles and boundaries across the care team and allows companions to remain focused on providing steady, in person support to the client.
