Our Case Management service provides structured, therapist-led coordination for individuals and families navigating mental health, addiction recovery, and complex medical systems through Camden Case Management. This service is designed to bring clarity, continuity, and steady guidance across care providers, daily needs, and family involvement.
Case Management does not replace therapy. It extends therapeutic goals into real-world environments through coordinated support, advocacy, and ongoing clinical oversight.

Case Management Service
We designed our Case Management services to be therapist-led because this work unfolds over time and across settings. Coordination and advocacy often intersect with symptoms, treatment plans, family dynamics, and daily stressors. Therapist-led oversight allows patterns to be noticed early and thoughtful adjustments to be made as situations evolve.
At Camden Case Management, licensed therapists provide clinical leadership and oversight for the work. They design the service model, guide the team, and collaborate with a client’s treating therapist when one is involved to ensure support remains aligned with therapeutic goals. This allows case managers to work within a broader clinical context rather than responding to isolated moments.
Throughout our work, the term therapist-led refers to this structure of clinical leadership and oversight. It does not mean that all service providers are licensed clinicians. Parent and Family Coaching is provided by licensed clinicians. Case managers, mentors, companions, and coaches work in clearly defined, non clinical roles, guided by therapist-led direction and support.
Case Management services are tailored to each client and family. Support may be lighter or more structured depending on what is clinically appropriate and practically needed.
Services may include:
- Ongoing care coordination across mental health, addiction, and medical providers
- Development of reintegration plans, including goals and next steps as clients transition between levels of care
- Oversight and coordination of Camden Case Management service providers, such as mentors, companions, and family and parent coaches, when involved
- Family and parent guidance to support informed, steady involvement

What Case Management Includes
Goal Tracking and Accountability
Case managers support collaborative goal setting and regular check-ins. This helps reinforce structure, follow-through, and alignment with therapeutic priorities.
Comprehensive Care Coordination
Coordination extends across therapy, medication management, inpatient hospitalization, outpatient and residential treatment programs, and specialty providers. Communication is structured to reduce fragmentation and support continuity of care.
Medical Advocacy and Navigation
Support may include assistance with navigating medical systems, coordinating appointments, supporting hospital discharges, and helping families understand care plans and insurance processes.
Parent and Family Case Management
Families and parents receive guidance, education, and advocacy. Case managers help families stay oriented, informed, and appropriately involved without replacing the therapeutic relationship.
Flexible Support Structure
Support is designed to balance structure with independence. Services may involve in-person, remote, or coordinated support depending on clinical needs and daily realities.

Each client and family receives a personalized support plan that integrates mental health, addiction recovery, and medical considerations. Plans are reviewed regularly through clinical supervision and adjusted as needs evolve.
Coordination of Camden Case Management Support Providers
When additional Camden Case Management services are involved, case managers oversee and coordinate the work of mentors, companions, and other support providers. This includes clarifying roles, aligning goals, and ensuring support remains consistent with the overall care plan and therapeutic priorities.
This coordination helps prevent duplication, reduces confusion, and allows families and clients to experience support as cohesive rather than fragmented.
How This Supports Therapeutic Work
All Case Management services at Camden Case Management are therapist-led and supported by weekly clinical supervision. Licensed therapists provide clinical leadership and oversight, working in alignment with a client’s treating therapist and broader care team. This allows therapeutic goals to be supported without replacing therapy.
Case managers work within clearly defined, non clinical roles under therapist-led guidance. They maintain ongoing communication with the clinical team, sharing relevant updates from the client’s day to day life, including how skills are being applied outside of sessions and any emerging stressors or changes that may affect care. Case managers also coordinate practical supports such as assessments, monitoring needs, and related logistics.
Within this structure, the case manager serves as a central point of coordination. They hold the full picture of the client’s goals, supports, and systems so care remains aligned and responsive as life unfolds.
