Case Management with mentor and companion support
Our Clinically-Informed Case Management with Mentor and Companion Support offers a flexible, comprehensive approach, providing clients with tailored assistance to meet their unique recovery needs. Through coordinated mentorship and companionship, clients receive a structured foundation designed to help them achieve their goals with the backing of a dedicated multidisciplinary team.
What This Includes:
- Companionship for Intensive Support with Step-Down Program: Designed for clients who need significant, hands-on assistance, companionship is available from 1 to 7 days per week, with options for 24, 18, or 12 hours per day. Our step-down program allows for a gradual reduction in hours as clients progress, easing the transition to independence with clinically guided adjustments.
- Mentorship for Moderate Support (Minimum of 2 Hours): Ideal for clients who are more independent but benefit from structured guidance, mentorship provides flexible support from 1 to 7 days per week, with sessions ranging from 2 to 11 hours per day. This option focuses on enhancing executive functioning, relational skills, and daily task management in a way that aligns with each client’s goals.
- Flexible, Weekly Adjustment: Both companionship and mentorship services are adaptable on a weekly basis, ensuring that the level of support can increase or decrease based on each client’s evolving needs, as guided by clinical recommendations.
- Personalized, Goal-Oriented Plans: Mentors and companions collaborate with clients to create individualized plans that address activities of daily living (ADLs), executive functioning, hobbies, and relational skills, ensuring alignment with the client’s overall treatment objectives.
- Clinical Oversight, Family Support, and Continuous Communication: Each client is supported by a multidisciplinary team, including Licensed Marriage and Family Therapists (LMFTs) and case managers who provide weekly supervision. As part of our commitment, families have access to a Family Support Group, offering resources and a supportive space to stay engaged in their loved one’s journey. Regular communication with the family and clinical team ensures all members are informed and aligned on the client’s needs, progress, and goals.
